Understanding Protected Health Information
It is our desire to safeguard your protected health information (PHI). PHI is any health information including patients past, present or future health care related services. This includes demographics, documentation of symptoms, examination and test results, diagnoses and treatments.
The Health Information Portability and Accountability Act of 1996 (HIPAA) Privacy Rule permits providers to use and disclose PHI without a patient’s written authorization for purposes of treatment, payment and health care operations. This allows us to send a patient’s PHI to continuing care providers and facilities and insurance companies to better assist in giving you a positive experience with our facility and foremost, your healthcare.
The HIPAA Privacy Rule permits use and disclosure of PHI without a patient’s authorization in other situations to include: public health activities, health oversight activities, judicial and administrative proceedings, decedent situations and research.
PHI may be used or disclosed for other purposes only with specific written authorization highlighted under the HIPAA Privacy Rule from the patient or patient’s representative.
We value your privacy in health and financial matters!
Please review the Notice of Privacy Practices for explanation on how we may use and disclose your health information and how you can get access to your medical records. The State of Oklahoma has developed a standardized form for patients to use in granting access to their medical records.
